<h2>A New Gout Drug. Febuxostat Wins Marketing Approval In The European Union</h2>

<p> It has taken a very long time for a major new gout drug to be developed and approved. Probably the best gout news in over 40 years, is that Febuxostat, which has been given the trade/brand name of Adenuric, has been approved by the European Medicines Agency for marketing in European Union (EU) countries. </p>
<p> This approval applies only to EU countries. Febuxostat has not yet been approved in the United States. If you are reading this from a non-EU country, or the US, you can check with your doctor to find out whether, or when, Febuxostat will be approved in your country. </p>
<p> Approval in one country does not mean approval in another, but the fact that the European Medicines Agency has approved it will carry weight when the medical authorities in other countries consider its approval.</p>
<p> <b> BENEFITS </b></p>
<p> In large trials Febuxostat (Adenuric) has outperformed Allopurinol, the most common gout medication for long term treatment. Like Allopurinol, it works to lower uric acid production by inhibiting xanthine oxidase, an enzyme required to produce uric acid. In two major studies it reduced uric acid (UA) in more gout sufferers, and more patients reached the target 6.0 mg/dL level more quickly, than did those on Allopurinol. </p>
<p> At 120 mg it has reduced UA levels to the target of 6.0 mg/dL in over 60% of study patients. At 80 mg the numbers were around 51%. Allopurinol at a 300 mg dosage in most participants achieved this is 22% of study patients. Hence the excited buzz in rheumatology circles since these kinds of results became apparent in its trials. At or below 6.0 mg/dL gout is more likely to be cured or managed better.</p>
<p> Generally, Febuxostat has been more effective when UA levels are below 10.0 mg/dL (595 mmol/mol). However, even if they have been above this level, it has reduced UA levels down to the target in about 40% of study patients at an 80 mg dosage, and by more at higher dosages. </p>
<p> <b> IT&#8217;S NOT PERFECT </b></p>
<p> In trials its success rate in reducing UA levels down to the important 6.0 mg/dL (357 mmol/mol) level was not 100%. About 35% of trial patients did not achieve the target. This may have been partly because so many gout sufferers are not UA over-producers, the problem Febuxostat addresses, but under-excretors of uric acid.</p>
<p> And it may cause side effects of course. See the bottom of this article. </p>
<p> Febuxostat (Adenuric) is not unfortunately a treatment for gout pain. It won&#8217;t be used instead of NSAIDS like Indomethacin, Colchicine, and corticosteroids. </p>
<p> It is recommended that NSAIDs or Colchicine are used along with Adenuric. This is because, as with Allopurinol, on a course of Febuxostat, there may be an initial increase in gout flares. If this happens you must battle through the attacks and continue taking Adenuric. </p>
<p> It will also only be used in gout patients who have chronic hyperuricemia (high UA levels) and gout.</p>
<p> So if you&#8217;re a gout sufferer who lives in a European country that&#8217;s a member of the EU, you can ask your doctor&#8217;s opinion about whether Febuxostat is suitable for you.</p>
<p> <b> WHO WILL BE PRESCRIBED IT?</b></p>
<p> The recommended dose is one 80 mg daily tablet. If after two weeks there is no reduction in UA levels, your doctor may increase the dose to 120 mg daily. Patients will need another serum UA test after two weeks treatment to find out how well it&#8217;s doing. In trials it has usually worked quickly, within a couple of weeks.</p>
<p> Your uric acid levels will have to be high. If you have gout but normal or slightly raised uric acid levels as some do, it is unlikely to be prescribed. </p>
<p> <b> Tophi </b> If you have tophi, it&#8217;s heartening to know that Febuxostat has been shown to have success against tophi. Measured tophi areas reduced in many patients with tophi. The presence of tophi will be an additional factor in favour of a decision to prescribe Febuxostat (Adenuric). </p>
<p> You will not need to be on a low purine diet, and more good news is that it can be prescribed for the elderly.</p>
<p> <b> Kidney and liver disease </b> In trials it has performed quite well with patients who have mild kidney disease. 44% of patients achieved the target at an 80 mg dosage, more at higher dosages. Better results than Allopurinol. If you have mild kidney disease (renal impairment) or mild liver problems (hepatic impairment) your doctor will judge whether Adenuric/Febuxostat can be taken safely.</p>
<p> If you have severe kidney or liver problems Febuxostat is not recommended since it has not been tested in these patient groups. It is not recommended for people with heart disease or children.</p>
<p> <b> Noted possible side effects </b> dizziness, drowsiness, diarrhoea, rashes, headaches, hypertension and liver function abnormalities considered mild ones. In trials none of these side effects were reported in more than 3.5% of trial patients. The most common were liver function abnormalities. </p>

<h2>Managing Gout And Uric Acid Kidney Stones</h2>

<p>Gout is painful enough, with uric acid forming crystals in the joints, but there are other &#8216;side effects&#8217; that can happen as well. These can be equally painful in many different ways, if not more so. Some people with gout end up with kidney stones, and if you know someone who has had them, you can figure out just how painful they can be by asking them how they managed. If you have gout, you may want to know how to fight off or deal with uric acid kidney stones that can form in your body.</p>
<p>Though calcium kidney stones are the most common, those with gout are at risk from kidney stones formed from an excess of uric acid. This means in order to prevent stones from forming, you have to watch your levels of uric acid.</p>
<p>You may already be doing this, but if you are not, this is all the more incentive for you to do so. Keeping animal proteins out of the diet as much as possible can do this, but there are other foods that can raise uric acid levels as well. It is a good idea to find a good gout diet and follow it to the letter if you want to avoid uric acid kidney stones.</p>
<p>As you&#8217;re probably already aware, excess uric acid in the body will form crystals that are deposited in the joints because it has no where else to go. As you may have guessed, it can also form the kidney stones. If you already have them, you have to work on getting rid of them, and then avoiding them in the future. Getting them out of your body is the hard, and sometimes, very painful part.</p>
<p>Some doctors will tell you two things if you are having a problem with uric acid kidney stones. One would be to drink more water along with following a low purine diet.</p>
<p>You should strive for at least 12 eight ounce glasses of water a day, or more if you can. That keeps your urine diluted, and lowers the risk of the crystals forming into a stone in your kidneys. This is a good idea no matter what your state of health, even if you don&#8217;t have gout. However, in this case, it is essential that you try and drink as much as you can. Please note, your water consumption should be spread out throughout the day and not just gulped in one or two sittings.</p>
<p>Another option is to take a medication called Polycitra K. Your doctor can tell you more about it. It is often taken when potassium levels are too low in the body, and the uric acid levels, as a result, are climbing. This medication should help stop the uric acid crystals from forming in the kidney before there is a problem.</p>
<p>If you already have kidney stones due to excess uric acid, what will happen next will be up to your doctor. The methods used for elimination often depend on your state of health, the size of the stones, and other considerations. There are times when the stones can be zapped with a laser, or similar device, so they break apart and will not be quite as painful when they come out, but there are also times when that is not possible, and they will have to be passed whole. Your doctor can help you with pain management, and plans to keep this from happening to you again in the future.</p>

<h2>Foods To Avoid With Gout While Dining Out</h2>

<p>Gout is a painful condition that is caused by a build up of uric acid crystals in joints. In most cases, this will happen in the big toe, or between the big toe and the one next to it. The overabundance of this uric acid will crystallize and settle in that joint and be pretty painful. Though gout is mostly a problem that men face, women develop gout on occasion. It is like arthritis in nature, but it can be very easy to treat with the right diet and certain medications. It&#8217;s a great idea for anyone with this condition to watch what he or she eats. Knowing what foods to avoid with gout will help greatly when you are eating out in a restaurant.</p>
<p>The foods that seem to irritate those with gout the most are those high in purines. These compounds are found in the human body, and in many different foods making them hard to avoid at times. So, when suffering from gout you need to be careful about the types of foods that you chose. Some of these purines won&#8217;t bother a person, but for others it leads to pain, and extreme pain at times. Knowing which foods to pass up can help you avoid the very painful episodes of gout that can leave you feeling incapacitated.</p>
<p>Some of the biggest problems for those with gout seem to come from eating meats and seafood. That is not good news for many, but avoiding these two food groups can help keep the pain down to a minimum. It is suggested that meat consumption be limited to six ounces a day and replacing proteins with eggs, and low fat milk foods as these are low in purines.</p>
<p>Some foods that are high in purines are okay to eat in moderation, for example peas, mushrooms, and beans. You shouldn&#8217;t eat huge quantities but you don&#8217;t have to limit them as much as you should limit meats and seafood.</p>
<p>Fats too must be limited, and foods fried in oils are never a good idea. Coffee and tea seem to be okay, but alcohol is not. When you add all of this together, eating at home can get complicated, but eating out can be even worse.</p>
<p>When you look through a menu, you may find that you don&#8217;t know what to get and what you should skip. The trick is to know what is in a dish, and also, how it is prepared. As someone who suffers with gout you may have to learn how to take the things you&#8217;ve learned at home and use them when ordering in a restaurant.</p>
<p>Someone with gout is going to have a hard time choosing something off a menu to eat, though this does depend on where this person is eating. It is always a good idea to ask how a dish is prepared, and ask for modifications to suit your gout friendly style of eating.</p>
<p>Most restaurants these days are very good at altering dishes to help those who might be dieting or who have specific medical conditions. Ask for the leaner varieties of your favorite dishes. Knowing what foods to avoid with gout will make it much easier to choose meals that are tasty to eat without causing you a painful gout flare-up. With the right modifications and knowing which foods to avoid with gout, a meal in a restaurant does not have to lead to pain and discomfort.</p>

But without going deeper into biochemistry, gout sufferers just need to know that raising body pH levels makes the body more alkaline, and lowering them, more acidic.</p>
<p> Raising your body&#8217;s pH levels is part of natural gout treatment. The theory is, supported by studies, that higher body pH levels (more alkaline) contribute to a reduction in uric acid levels because uric acid (diluted from drinking lots of water) becomes more soluble if the body is more alkaline. The higher the acid/alkaline ratio i.e. the higher the pH number on the pH scale, the more alkaline your body, and the more uric acid you should dissolve.</p>
<p> If you reduce your uric acid levels to below 6.0 mg/dL (men), slightly less in women, it is more likely that the troublesome MSU crystals will dissolve and a gout cure will be achieved.</p>
<p> ACHIEVING A MORE ALKALINE BODY</p>

So small number changes on this scale in fact mean very significant differences between the alkalinity and acidity of different foods.<br />

And it&#8217;s not the word off the lips of celebrity chefs. But you can find the pH of foods and drinks on the Internet. <h2>Delicious Low Purine Foods For Gout Sufferers</h2>

<p>Under normal circumstances, the purines you find in many foods are broken down into uric acid, and the body gets rid of it without any problems. However, for someone with gout, getting rid of this uric acid is problematic, and the result is painful crystals forming in the joints &#8211; leading to gout pain. When this happens, there are medications that can help with the pain, but one of the best things a person can do it to is to find low purine foods and make them a major part of their diet. This can offer much-needed relief for most.</p>
<p>There are some general directions for a low purine diet, but always listen to your doctor&#8217;s advice in addition to this information. A low purine diet is going to contain almost fifty percent carbohydrates each day, and no more than thirty percent of the diet each day should be made up of fats. The rest should be healthy and lower purine protein sources. Drinks that are normally not allowed on many special diets are actually encouraged. These include soda and coffee. The reason being as that they act as directics, helping your body to flush out the excess uric acid. It&#8217;s important to drink plenty of water as well to keep your body well hydrated. Alcohol, however, in most forms is not something you should be drinking.</p>
<p>When looking for low purine foods for your diet, you can find a great variety of things you can have that won&#8217;t cause any issues with your gout. If you love thick soups, you can have them as long as they are made with low fat milk. Cheese and milk are fine, but again, they should be low in fat. You can also enjoy things like gelatin and a great variety of fruits and fruit juices. Most sweets are just fine, but remember to watch the calories if you are attempting to get your weight down. You can also enjoy things like pasta, and peanut butter is okay for a treat.</p>
<p>When choosing your low purine foods, you can have some medium purine rated foods with them about once a day if it does not bother you. Those would include some meats like beef, pork, and possibly chicken. Some seafood choices are relatively safe as well. You should experiment to find what bothers you and what works out well. As long as you work at limiting the purines, you are going to feel much better each day. You might want to have lobster bisque on occasion, as long as you limit your portions, and don&#8217;t have any other medium purine foods for the rest of the day.</p>
<p>You may have a hard time finding new foods to eat at first, but after a while you will get used to reading labels and figuring out what works for you and what does not. Any special diet requires a little thought and work, and the low purine foods diet is no exception. After a while though, reading and deciphering items in the store by reading through the ingredients list will become second nature, and you will become accustomed to knowing how to build your own menu with little thought. Feeling better should be all the motivation you need.</p>
