<h2>A New Gout Drug. Febuxostat Wins Marketing Approval In The European Union</h2>
</div>
<p> It has taken a very long time for a major new gout drug to be developed and approved. Probably the best gout news in over 40 years, is that Febuxostat, which has been given the trade/brand name of Adenuric, has been approved by the European Medicines Agency for marketing in European Union (EU) countries. </p>
<p> This approval applies only to EU countries. Febuxostat has not yet been approved in the United States. If you are reading this from a non-EU country, or the US, you can check with your doctor to find out whether, or when, Febuxostat will be approved in your country. </p>
<p> Approval in one country does not mean approval in another, but the fact that the European Medicines Agency has approved it will carry weight when the medical authorities in other countries consider its approval.</p>
<p> <b> BENEFITS </b></p>
<p> In large trials Febuxostat (Adenuric) has outperformed Allopurinol, the most common gout medication for long term treatment. Like Allopurinol, it works to lower uric acid production by inhibiting xanthine oxidase, an enzyme required to produce uric acid. In two major studies it reduced uric acid (UA) in more gout sufferers, and more patients reached the target 6.0 mg/dL level more quickly, than did those on Allopurinol. </p>
<p> At 120 mg it has reduced UA levels to the target of 6.0 mg/dL in over 60% of study patients. At 80 mg the numbers were around 51%. Allopurinol at a 300 mg dosage in most participants achieved this is 22% of study patients. Hence the excited buzz in rheumatology circles since these kinds of results became apparent in its trials. At or below 6.0 mg/dL gout is more likely to be cured or managed better.</p>
<p> Generally, Febuxostat has been more effective when UA levels are below 10.0 mg/dL (595 mmol/mol). However, even if they have been above this level, it has reduced UA levels down to the target in about 40% of study patients at an 80 mg dosage, and by more at higher dosages. </p>
<p> <b> IT&#8217;S NOT PERFECT </b></p>
<p> In trials its success rate in reducing UA levels down to the important 6.0 mg/dL (357 mmol/mol) level was not 100%. About 35% of trial patients did not achieve the target. This may have been partly because so many gout sufferers are not UA over-producers, the problem Febuxostat addresses, but under-excretors of uric acid.</p>
<p> And it may cause side effects of course. See the bottom of this article. </p>
<p> Febuxostat (Adenuric) is not unfortunately a treatment for gout pain. It won&#8217;t be used instead of NSAIDS like Indomethacin, Colchicine, and corticosteroids. </p>
<p> It is recommended that NSAIDs or Colchicine are used along with Adenuric. This is because, as with Allopurinol, on a course of Febuxostat, there may be an initial increase in gout flares. If this happens you must battle through the attacks and continue taking Adenuric. </p>
<p> It will also only be used in gout patients who have chronic hyperuricemia (high UA levels) and gout.</p>
<p> So if you&#8217;re a gout sufferer who lives in a European country that&#8217;s a member of the EU, you can ask your doctor&#8217;s opinion about whether Febuxostat is suitable for you.</p>
<p> <b> WHO WILL BE PRESCRIBED IT?</b></p>
<p> The recommended dose is one 80 mg daily tablet. If after two weeks there is no reduction in UA levels, your doctor may increase the dose to 120 mg daily. Patients will need another serum UA test after two weeks treatment to find out how well it&#8217;s doing. In trials it has usually worked quickly, within a couple of weeks.</p>
<p> Your uric acid levels will have to be high. If you have gout but normal or slightly raised uric acid levels as some do, it is unlikely to be prescribed. </p>
<p> <b> Tophi </b> If you have tophi, it&#8217;s heartening to know that Febuxostat has been shown to have success against tophi. Measured tophi areas reduced in many patients with tophi. The presence of tophi will be an additional factor in favour of a decision to prescribe Febuxostat (Adenuric). </p>
<p> You will not need to be on a low purine diet, and more good news is that it can be prescribed for the elderly.</p>
<p> <b> Kidney and liver disease </b> In trials it has performed quite well with patients who have mild kidney disease. 44% of patients achieved the target at an 80 mg dosage, more at higher dosages. Better results than Allopurinol. If you have mild kidney disease (renal impairment) or mild liver problems (hepatic impairment) your doctor will judge whether Adenuric/Febuxostat can be taken safely.</p>
<p> If you have severe kidney or liver problems Febuxostat is not recommended since it has not been tested in these patient groups. It is not recommended for people with heart disease or children.</p>
<p> <b> Noted possible side effects </b> dizziness, drowsiness, diarrhoea, rashes, headaches, hypertension and liver function abnormalities considered mild ones. In trials none of these side effects were reported in more than 3.5% of trial patients. The most common were liver function abnormalities. </p>

<h2>Adjusting Your Diet For Gout? &#8211; What Foods Contribute To Gout?</h2>

<p>I have gout. And my first gout attack was a very painful introduction to a condition that is more common than most people think. It completely blindsided me. I had no idea that my diet (and a few other factors) had been putting me at risk for gout. Who knows how long this disease had been stalking me ready to strike. I have since learned that a diet loaded with foods high in <em>purines</em> can cause and aggravate gouty arthritis (gout is a type of arthritis).</p>
<p><strong>Just a short story about how my first gout attack happened:</strong></p>
<p>I was on vacation with my wife and we were staying at a hotel in Denver, Co. We were having a good time and splurging; a night on the town (a few beers), a nice meal (shrimp cocktail, prime rib and creamed spinach), we were having a great time (a few more beers). </p>
<p>I popped some aspirin because I always take aspirin for anything that ails me (I heard it is good for hangovers too), and I took my high blood pressure medication (a diuretic). The next morning I woke up and my right foot had swollen up so big that I could not even get my shoe on. And talk about PAINFUL, it virtually took me 5 minutes to put a sock on and I could barely walk! </p>
<p>I thought to myself, &#8220;What the hell is going on?&#8221; So I popped some more aspirin for the pain (Oh boy, big mistake &#8211; I&#8217;ll explain later). Long story short, I had to go see a doctor and he told me I was having a gout attack. When he listed the dietary risk factors for gout, I immediately recognized that a lot of the foods I had been eating coupled with some drugs I was taking (and being overweight did not help matters) had made me a walking &#8220;gout time bomb.&#8221;  <strong><br />
Here is a short breakdown of some of the dietary factors which contributed to my gout attack: </strong></p>
<p>1) <strong>Seafood:</strong> Although the shrimp I had the night before my attack was undeniably a factor because all shellfish have high purines, shrimp is not the worst offender. Some other fish have even higher amounts of purines &#8211; anchovies, sardines, smelt, and mussels are just a few. Back off the seafood if you have gout.</p>
<p>2) <strong>Red Meat:</strong> I had a huge slab of prime rib the night before my attack. It was delicious, but red meat is high in purines, so it was a contributing factor to my gout attack. Liver, sweetbreads, organ meats, and gravies are some other foods that are in this high purine category.</p>
<p>3) <strong>Alcohol:</strong> Beer is a big No No if you have gout, which pisses me off (because I love it). I had probably 8-10 beers the night before my gout attack. All alcoholic drinks can aggravate gout, but beer is especially bad because it has large amounts of brewers yeast and yeast is one of the highest purine containing foods there is. If you have gout, just moderate your drinking and you should be ok.</p>
<p>4)<strong> Some veggies are high in purines: </strong>Spinach is one of them, and I did have spinach the night before my attack (culminating in my perfect storm for a gout attack). Some other high purine vegetables are asparagus, cauliflower, mushrooms, kidney and lima beans. Please note that these are just moderately high in purines and most likely will not be a problem to gout sufferers in moderation. Most vegetables are actually very low in purines.</p>
<p>Ok the dietary breakdown of my &#8220;night before&#8221; is completed. But there are some other things I was taking that absolutely contributed to my gout attack. Remember I said I would explain about aspirin? Well, aspirin is also a contributing factor for gout when taken in high amounts. Aspirin increases the level of uric acid in the body and uric acid crystals in the joints of the body is what causes gout. I was taking probably 8-10 aspirin/day before my gout attack, I have since changed over to ibuprophen. Low dose aspirin in most cases will not aggravate gout. </p>
<p>And last, but not least, I have high blood pressure, am overweight, and in my 50&#8242;s. I was taking a high blood pressure medication which contained a diuretic. Diuretics have been shown to be associated with gout because they can also increase the uric acids levels in the system. Although not the main culprit in most gout attacks, diuretics can be a contributing factor. </p>
<p>Over the years I have learned a lot about what causes gout, the proper diet to decrease my chances of having a gout attack, and how to live with this painful disease. At the time of this writing it has been nearly 3 years since I have had an attack of gout and I give credit in large part to a proper diet. If you have gout and want to lessen your attacks then you should also learn how your diet can be adjusted to eliminate gout from your life. Believe me, it is a lot less painful to change your diet for gout, than to suffer with gout.</p>

<h2>Gout is Caused by Too Much Uric Acid in the Blood</h2>

<p>Gout is a form of arthritis that attacks of sudden pain, stiffness and swollen joints. That gout attacks may continue to return until finally being treated. They are mostly men who suffer from gout; the disease is much less common in women. It is not clear why men especially vulnerable to gout. The attacks bring the joints associated with arthritis and tissue damage, so the symptoms of the disease are going to be worse.</p>

The chances of gout are increased in case of overweight, drinking too much alcohol or eating lots of meat and fish. Some medicines can cause gout as a side effect also. Most common symptom of gout is a nighttime attack by a swollen big toe, assault involving a sharp pain and reddening of the toe. Such attacks can also emerge in the foot, ankle or knee. They may be a few days to a few weeks before they disappear.</p>
<p>The attacks can come back regularly or they may also disappear. In some cases, months or even years before a new attack occurs. It can not hurt to consult a doctor, even though the pain associated with gout disappeared. <p>First, the regular consumption of organ meats &#8211; liver, kidneys, brains and so on &#8211; indeed a factor contributing to the prevention of gout. These bodies contain a high content of purine, a substance that is considered partly responsible for gout. But the study showed that diet is not the main responsible for the development of gout should be counted.</p>

<p>Acute attacks of gout can be treated with a number of different medications including low doses of colchicines, non-steroidal anti-inflammatory drugs (NSAIDS), and glucocorticoids (steroids) given either by mouth, by vein, or into the joint. It is generally not difficult to break an acute attack.</p>
<p>The big problem is when it comes to chronic attacks and chronic arthritis. Because it is important not only to help with the painful symptoms of arthritis but also to find away to lower the amount of uric acid in the system.</p>
<p>To date, only two primary drugs are available for reducing uric acid.</p>
<p>Probenecid works by making a person urinate out more uric acid. This drug has three drawbacks. The first is that if a patient is already putting out a lot of uric acid in the urine (and a doctor should check this with a 24 hour urine collection before starting a patient on probenecid), then the patient is at increased risk for developing kidney stones. The second issue with probenecid is that it works only in patients with relatively normal kidney function. The final problem is that if a patient has a lot of uric acid in their system and they have large collections (called &#8220;tophi&#8221;) in and around their joints, probenecid is not usually strong enough to have a beneficial effect. One other drawback is that probencid has some drug interaction with medicines such as penicillin.</p>
<p>The second drug that is used is allopurinol. This is a purine xanthine oxidase inhibitor which means it works by reducing the production of uric acid. While it works well for many patients, particularly those with tophaceous gout, there are many drawbacks. These include drug interactions with multiple medications as well as other potentially serious toxicities. Allopurinol causes severe skin rashes, liver and kidney damage, as well as bone marrow problems. Allergic reactions can cause death.</p>
<p>Therefore, there is an unmet clinical need for new gout treatments. Even when patients are given desensitization treatments to try and help them take allopurinol, these treatments often fail.</p>
<p>Unfortunately, other treatment options are relatively limited. To lower uric acid, removing contributing factors such as medications, where possible, can be useful. For example, thiazide diuretics are a common cause of elevated uric acid.</p>
<p>Alteration in diet can be beneficial, but compliance is often difficult for many patients.</p>
<p>Other medications with uric acid-lowering properties, such as losartan (Cozaar) and fenofibrate (Tricor), can be tried if there are no contraindications.</p>
<p>A drug currently in late-phase development, febuxostat, is a non-purine xanthine oxidase inhibitor that has shown promise in clinical trials but has not yet been approved for gout. It can be used in patients who are allergic to allopurinol.</p>
<p>PEG-uricase is another drug in development. It converts uric acid into allantoin, a substance that does not induce inflammation.</p>
<p>There is anecdotal evidence suggesting that IL-1 inhibitors, drugs that are currently being used for other inflammatory types of arthritis such as rheumatoid arthritis, may be effective in gouty arthritis, and this approach is currently under study.</p>
<p>Nonsteroidal anti-inflammatory drugs (NSAIDs), in addition to being used for acute attacks, are commonly used for the treatment of chronic arthritis.</p>
<p>Comorbid conditions commonly found among patients with gout, including hypertension and kidney disease, which in some cases contribute to the disease, can limit the use of NSAIDs in patients with gout, however.</p>

<p>The general public doesn&#8217;t know it, but some gout sufferers do. At long last we are in a good period for new gout treatments. In fact gout treatment looks rosier now than ever before in the known history of the disease. And that&#8217;s over 2,000 years.</p>
<p>Febuxostat has been approved by the European Medicines Agency (EMEA) for sale in EU countries, and many other countries should follow, although it has not been approved in the U.S. and it&#8217;s not known at present when it will be. The developers of pegloticase will apply later in 2008 to the FDA for U.S. approval.</p>
<p>And at the European League Against Rheumatism (EULAR) meeting in Paris in June 2008 new anti-gout compounds, currently called RDEA806 and RDEA594, were presented. In an early human study they have demonstrated potent reduction of uric acid (UA), the excess of which is a cause of gout in most cases. RDEA594 will be developed as a uric acid reducing drug medication, but it has many years of trials ahead of it before approval can come.</p>
<p><strong>TOPHI GOUT</strong> <strong>TREATMENT </strong></p>
<p>One of the most encouraging things about febuxostat (EU trade name: Adenuric) and pegloticase is their ability in many cases to dissolve<strong> </strong>tophi. Tophi are the unsightly lumps of MSU crystals which develop in the bodies of gout sufferers after several years if there is no gout cure or sufficient alleviation to stop tophi growing.</p>
<p>They usually first appear in toes, but also in hands, ankles, elbows, knees, body tissues and other locations. Even inside the kidneys and heart, although this is rare. They may be benign, they may not. If they aren&#8217;t, they can burst out of the skin, become ulcerated and septic &#8211; perhaps leading to that dangerous condition sepsis (septicemia) &#8211; destroy joints and grow into bone. Their appearance in long term gout sufferers is not inevitable. About 25% of all gout sufferers have some degree of tophi.</p>
<p>Tophi&#8217;s growth can be halted, and tophi dissolved, if blood uric acid levels can be reduced to the 6 mg/dL,(357 mmol/L)or below, mark. Studies have shown that this frequently happens at or below this level.</p>
<p>Short of surgical removal, which does sometimes become necessary to stop the problems noted above, lowering uric acid levels is the only way of dealing with tophi.</p>
<p>In phase 3 trial extensions both febuxostat and pegloticase have been successful against tophi, but success has taken time. Febuxostat had little effect after six months and the EMEA&#8217;s assessors were not clear that it had been very successful after one year. But after a two year trial extension (i.e. three years in total) febuxostat delivered a complete tophi removal success rate of over 50%. Pegloticase, for which less data is currently available than for febuxostat, and which has not been approved anywhere yet, has also been more successful in trial extensions.</p>
<p><strong>HOW LONG DOES IT TAKE?</strong></p>
<p>Much depends on the size, number and location of tophi in the body, patient compliance with treatment, and starting treatment UA levels. Febuxostat has so far shown most success after three years of treatment. And the statements the developers of pegloticase have made also suggest that the longer the treatment, the more effective against tophi it will be.</p>
<p><strong>NATURAL GOUT REMEDIES </strong></p>
<p>The addition of natural gout remedies &#8211; diets, special foods such as cherries, drinking water as if you are a gas guzzling car, improving body pH levels with baking soda (bicarbonate of soda) and other alkalizing agents, and vitamins, has not been tested with these medications. But I suspect that, <em>if it is safe for them to be used, </em>they will be of extra help, and so play a part in treating <strong>tophaceous gout,</strong> maybe faster than the drug medications alone.</p>
